The Beautiful People singer was injured during a concert at the Hammerstein Ballroom, in New York, after he climbed onto the giant pistols stage prop.
As the 48-year-old pulled himself up the set piece gave way and 'toppled' onto him.
Fans screamed while several people rushed to help Manson who was stretchered off stage.
Michelle Curran, who was at the show, told Mail Online: "He was probably half way through singing Sweet Dreams by the Eurythmics and the massive gun display of two pistols behind him toppled on to him.
"The singing stopped and loads of people ran to the stage to life it up."

The rest of the gig was cancelled.
Manson's team has yet to release a statement about his condition.
The award-winning star was promoting his new album, Heaven Upside Down.

Manson recently feuded with Justin Beiber after the Canadian pop star wore a T-shirt featuring the black-haired singer's name and face on the front and the text "Bigger Than Satan".
He claimed Justin had used his likeness without his permission for his Purpose tour merchandise.
